The procedure for obtaining an accreditation
certificate consists of 2 stages:

Submitting an application to the Ministry of Health to obtain permission for an accreditation inspection.

Preparation of all necessary documentation. Passing an inspection by a healthcare facility and obtaining a certificate.

List of required documents:

A copy of the institution’s regulations (charter) certified in accordance with the procedure established by law.
A copy of the licence(s) (if any) for conducting business activities in medical practice, manufacturing of medicinal products, wholesale and retail trade in medicinal products.
Copies of documents confirming the right to use the premises and equipment necessary for the provision of medical care, certified in accordance with the established procedure.
Approved structure of the healthcare facility.
Report on medical staff (in the form approved by the order of the Ministry of Health of Ukraine).
The conclusion of the state sanitary and epidemiological examination issued at the place of business on the compliance of the institution, except for pharmaceutical (pharmacy) institutions, with the requirements of sanitary norms and rules.
Report of the institution on the medical work carried out over the past three years, and in case of the first accreditation
for the last two years (in the form approved by the order of the Ministry of Health, according to the type of institution).
An analysis of the institution’s performance in terms of quality of care and criteria for its assessment.
A copy of the certificate of certification of clinical diagnostic and/or control and analytical and other measuring laboratories (if they are available in the structure of the institution).
Copies of previously issued accreditation certificates, if any.
