LIST OF REQUIRED DOCUMENTS

TO COMPLETE THE APPLICATION, YOU MUST SPECIFY:

name of the product (trade name in the original language, English and Ukrainian);
name of the applicant (address, telephone, fax, e-mail address), for foreign applicants – registration number, for domestic applicants – code according to the EDRPOU;
name of the manufacturer (address, telephone, fax, e-mail address), for foreign manufacturers – registration number, for domestic manufacturers – code according to EDRPOU;
composition of the product, dosage form, packaging;
purpose of the product;
mode of use;
for imported medicinal products – code according to UKTZED (2371a-14, 2371b-14, 2371c-14, 2371d-14);
for domestic medicinal product – code according to the UKPD;
a document certifying that the applicant is authorised to represent the interests of the manufacturer (if the applicant is not the manufacturer).

State registration (re-registration) of medicinal products is carried out by the Ministry of Health of Ukraine.

State registration (re-registration) is required for products subject to hygiene standards.

The validity period of the state registration (re-registration) of a medicinal product is five years, which may be reduced at the applicant's request.

Re-registration of a medicinal product is carried out by the Ministry of Health in the same manner as its registration:

expiration of the state registration of the product;

changes in the composition of the product;

changing the name of the product;

transfer of rights to manufacture the product to another manufacturer;

changes in the intended use of the product.
